| | | | | | | | PROJECT DESCRIPTION: The agreement between the City of Buckeye and OptumHealth Care Solutions, LLC, will allow members of OptumHealth Care Solutions to utilize the fitness equipment and programs at the Buckeye Community Center as a participating network facility within the OptumHealth Newtwork. In return for use of Buckeye’s fitness equipment and programs, OptumHealth Care Solution, Inc. will reimburse the City funds based on the amount of visits per person from eligible participants enrolled in the Fitness Passport Program.

Approval of the this agreement allows the City of Buckeye’s Community Center to be a participating network facility for the Optum Fitness Passport Program and will allow the City to receive additional revenue. The program will provide Buckeye residents enrolled in OptumHealth another convenient location to exercise and improve their health and wellness.

Upon approval of this agreement, staff can proceed with executing the agreement with OptumHealth Care Solution, LLC. Once executed, City staff will begin tracking visits from participants in the program to submit to OptumHealth for reimbursement.

| | | | | | | | FINANCIAL IMPACT STATEMENT: Must be completed before submission
The Community Center will not incur any additional cost. The City will be reimbursed a rate of $4.00 per visit with a maximum of $32 per person per month for Optum Fitness Passport members. Revenues from the program will be deposited into GL#10004160 435400 and budgeted in the FY 18/19 budget.
